The forms on this website are for reference only and should not be completed by participants. 

Some of the questionnaires have been revised over recent years. Researchers who need a complete overview of all forms are asked to contact .

Invitation and statements of consent

During pregnancy, mothers received an invitation to participate in MoBa. The invitation and statements of consent are only available in Norwegian. 

Week 15 of pregnancy

This questionnaire was sent out in the 15th week of pregnancy. Mothers were asked about their mental and physical health in the first part of pregnancy and before conception. The questionnaire sent to the father concerned the father's physical and mental health, lifestyle and working conditions. Collection of these questionnaires is complete.

Week 22 of pregnancy

This form was sent out in the 22nd week of pregnancy and was related to the mother’s diet in pregnancy. The form could also be filled out electronically. Collection of this questionnaire is complete.

Week 30 of pregnancy

The questionnaire was sent out in 30th week of pregnancy and concerned the mother's mental and physical health at this stage of pregnancy. Collection of this questionnaire is complete.

6 months after birth

The main themes are maternal physical and mental health around the birth and the time immediately after, as well as the child’s health, nutrition and general development. Collection of this questionnaire is complete.

18 months after birth

The questionnaire was sent out when the child was 18 months old and was similar to questionnaire at 6 months so changes could be measured. Collection of this questionnaire is complete.

36 months after birth

The questionnaire was sent out when the child was 36 months old and contained questions about the child’s mental and physical development, and questions about maternal health and lifestyle. Collection of this questionnaire is complete.

5-year-olds

This questionnaire was sent out when the child was 5 years old and contained questions about neurodevelopmental disorders and language delays. Collection of this questionnaire is complete.

Questionnaire to childcare centres 

This questionnaire was sent to some families when the child was 5 years old and contained questions about activities and situations in the childcare centres. The parents were asked to bring the form to the childcare centres to be filled out by the childcare employees. The form was only sent to parents with children born after 2006. Data from the childcare centre questionnaire can currently only be used in co-operation with the Language and Learning Study (SOL). Collection of this questionnaire is complete.

7-year-olds

This questionnaire was sent out when the child was 7 years old and contained questions about the child’s health, lifestyle and nutrition. Collection of this questionnaire is complete.

8-year-olds

This questionnaire was sent out when the child was 8 years-old and focused on social and mental development in children. Collection of this questionnaire is complete.

Questionnaire to schools

Two questionnaires was sent out during the child’s school years. Both were completed by the child’s teacher. The forms contained questions about the pupils learning environment and how the pupil is doing in school and in general. Data from the school questionnaire can currently only be used in cooperation with The Language and Learning Study (SOL). Collection of these questionnaires are complete. These questionnaires only exist in Norwegian. The instrument documentation will soon be available in English. 

13-year-olds

MoBa wants to find out more about what young people eat. We have little knowledge of young people's eating habits and how diet affects their health now and later in life. Collection is ongoing. 

14-year-olds

This questionnaire is sent out when the child is 14-years-old. The form contains questions about health and lifestyle, as well as adolescent behaviour. Collection is ongoing.

Variables from these questionnaires are not available for extraction, please  if further information is needed. 

16 to 17-year-olds (MoBaUng)

MoBa wants to find out more about the youth’s wellbeing, and which conditions during adolescence are of importance for health in adulthood. These questionnaires only exist in Norwegian. The instrument documentation will soon be available in English. Collection is ongoing.

Currently, only variables from questionnaire 1 and 2 are available for extraction. Please if further information is needed. 

Questionnaire for fathers 2015

In December 2015 we sent out a questionnaire to the fathers. At first, we sent it to the fathers who participate with one child in MoBa. Fathers who participate with several children received the questionnaire in 2016. Collection of this questionnaire is complete.

Other forms

Questionnaires about influenza and vaccine for mother and child

In the spring of 2010, MoBa asked all participants to fill out a questionnaire about influenza and vaccination. The form was sent out regardless of the child’s age. Collection of these questionnaires is complete. These questionnaires only exist in Norwegian. The instrument documentation will soon be available in English.

Questionnaire about coronavirus (COVID-19) for parents and children

In March/April 2020, MoBa began sending out questionnaires about the coronavirus to all participants. As of November 2022, close to 60 questionnaires have been sent out to monitor the prevalence and burden of disease. Collection is still ongoing. Variables from these forms are now available on request, but they are not yet organized in a way that makes them suitable for disclosure. This makes the data demanding to extract, and it must therefore be noted that delivery can take a long time, especially in cases where a large number of variables are needed. We recommend starting with a smaller number of variables. Please contact the MoBa administration to receive an overview of the variables available.
