This page contains invitation, statements of concent, questionnaires and instrument documentation for the Norwegian Mother, Father and Child Cohort Study (MoBa).
Please note that the forms on this website are for reference only and should not be completed by participants.
In MoBa, we have collected data from both first‑generation participants (mothers and fathers) and second‑generation participants (children). The questionnaires are therefore grouped according to who completed them, and for whom they were completed. Variables collected from subsets of the MoBa cohort can be found at the bottom of this page.
Some of the questionnaires have been revised over recent years. Researchers who need a complete overview of all forms are asked to contact the MoBa administration.
Invitation and statements of consent
During pregnancy, mothers received an invitation to participate in MoBa. The invitation and statements of consent are only available in Norwegian.

Questionnaires filled out by the first-generation participant during pregnancy
Questionnaire 1 (Q1) – filled out by mothers around week 15 of pregnancy
This questionnaire was sent out in the 15th week of pregnancy. Mothers were asked about their mental and physical health in the first part of pregnancy and before conception. Collection of these questionnaires is complete.
Father's Questionnaire (QF) – around week 15 of pregnancy
The questionnaire sent to the father concerned the father's physical and mental health, lifestyle and working conditions. Collection of these questionnaires is complete.
Questionnaire 2 (Q2) – filled out by mothers around week 22 of pregnancy
This form was sent out in the 22nd week of pregnancy and was related to the mother’s diet in pregnancy. The form could also be filled out electronically. Collection of this questionnaire is complete.
Questionnaire 3 (Q3) – filled out by mothers around week 30 of pregnancy
The questionnaire was sent out in 30th week of pregnancy and concerned the mother's mental and physical health at this stage of pregnancy. Collection of this questionnaire is complete.
Questionnaires filled out by the first-generation participant about themselves and the child
Questionnaire 4 (Q4) – filled out by mothers when the child is 6 months
The main themes are maternal physical and mental health around the birth and the time immediately after, as well as the child’s health, nutrition and general development. Collection of this questionnaire is complete.
Questionnaire 5 (Q5) – filled out by mothers when the child is 18 months
The questionnaire was sent out when the child was 18 months old and was similar to questionnaire at 6 months so changes could be measured. Collection of this questionnaire is complete.
Questionnaire 6 (Q6) – filled out by mothers when the child is 3 years
The questionnaire was sent out when the child was 3 years old and contained questions about the child’s mental and physical development, and questions about maternal health and lifestyle. Collection of this questionnaire is complete.
Questionnaire 5 years (Q5Y) – filled out by mothers
This questionnaire was sent out when the child was 5 years old and contained questions about neurodevelopmental disorders and language delays. Collection of this questionnaire is complete.
Questionnaire 7 years (Q7Y) – filled out by mothers
This questionnaire was sent out when the child was 7 years old and contained questions about the child’s health, lifestyle and nutrition. Collection of this questionnaire is complete.
Questionnaire 8 years (Q8Y) – filled out by mothers
This questionnaire was sent out when the child was 8 years-old and focused on social and mental development in children. Collection of this questionnaire is complete.
MoBa Youth Questionnaire 14 years (Q14M) – filled out by mothers
This questionnaire is sent out when the child is 14 years old. The form contains questions about health and lifestyle, as well as adolescent behaviour. Collection is ongoing.
Father's Questionnaire no. 2 (QF2) – filled out by fathers 2015 - 2016
In December 2015 we sent out a questionnaire to the fathers. At first, we sent it to the fathers who participate with one child in MoBa. Fathers who participate with several children received the questionnaire in 2016. Collection of this questionnaire is complete.
Questionnaires filled out by the second-generation participant about themselves
Dietary Questionnaire 13 (Q-youth diet) – 13-15 years
MoBa wants to find out more about what young people eat. We have little knowledge of young people's eating habits and how diet affects their health now and later in life. Collection of this questionnaire is complete.
MoBa Youth Questionnaire 14 years – Child (Q14C)
This questionnaire is sent out when the child is 14 years old. The form contains questions about health and lifestyle, as well as adolescent behaviour. Collection is ongoing.
MoBa Ung Questionnaires – 16- to 17-year-olds
MoBa wants to find out more about the youth’s wellbeing, and which conditions during adolescence are of importance for health in adulthood. These questionnaires only exist in Norwegian. The instrument documentation will soon be available in English. Collection is ongoing.
Currently, only variables from questionnaire 1,2 and 3 are available for extraction. Please contact the MoBa administration if further information is needed.

18-Year Questionnaire
In this survey we wish to learn more about the health and wellbeing of 18-year-olds, whether they have been infected with covid and their experiences when Norway went into lockdown. This questionnaire is part of MoBa’s collection and is dispatched once after participants have turned 18 years. Collection is ongoing.
19-Year Questionnaire
In this survey we wish to learn more about the current living situation of 19-year-olds, their educational and career aspirations and their thoughts on the future. This questionnaire is part of MoBa’s collection and is dispatched once after participants have turned 19 years. Collection is ongoing.
20-year Questionnaire
In this survey, we want to learn more about what affects the health of young adults in Norway today. The purpose of this questionnaire is to gain increased knowledge about how lifestyle and environment impact and have impacted the health, puberty, and development of young adults today.
Ung Helse/Young health
Ung Helse/Young Health is aimed at participants who are young adults (18–30 years old). The purpose is to gain knowledge about health, lifestyle, and life choices among young people, and to map well-being, life situation, and factors that influence health. The survey was sent out for the first time in 2024.
Ung Helse 1/Young Health 1
In this survey, young people are asked about life choices, family and the future.
Ung Helse 2/Young Health 2
This questionnaire is a follow-up sent to those who have already responded to Young Health 1. The questionnaire was first sent out in 2025
Health in Pregnancy
This is a follow‑up questionnaire that participants are presented with immediately after completing Young Health 1, Young Health 2, the 19‑year questionnaire, or the 20‑year questionnaire, if they have indicated in the respective questionnaire that they are pregnant. The purpose is to collect information about health, lifestyle, and experiences during pregnancy.
Life choices and thoughts about children
The respondents of this survey are those who have completed the Young Health questionnaire. The aim is extending the knowledge of the thoughts of young people about life choices and attitudes towards having children. There may be some overlap in questions between this and Young Health. Repeating questions may extend the knowledge about changing attitudes over the course of time. Some participants are shown a information video before completing the questionnaire.

Parent's 45-year Questionnaire
In this survey, parents are asked to contribute to new knowledge about how fertility affects health throughout life. The form is part of MoBa's collections and both father and mother will receive a questionnaire after they have reached the age of 45. Collection is ongoing.

Women's health Questionnaire to MoBa mothers
Questionnare sent to all Moba mothers. Questions revolve around children and family life, menstruation, menopause and issues after childbirth.
Health Questionnaire to MoBa fathers
Questionnaire sent to all Moba fathers. Questions revolve around children and family life, health issues, sleep, nutrition and other lifestyle factors. The first invitations were sent 02.09.24, and the questionnaire is still ongoing.
Other questionnaires in MoBa
Questionnaires about influenza and vaccine for mother and child (MoBaFlu)
In the spring of 2010, MoBa asked all participants to fill out a questionnaire about influenza and vaccination. The form was sent out regardless of the child’s age. Collection of these questionnaires is complete. These questionnaires only exist in Norwegian. The instrument documentation will soon be available in English.

Questionnaire about coronavirus (COVID-19) for parents and children
In March/April 2020, MoBa began sending out questionnaires about the coronavirus to all participants. As of November 2022, close to 60 questionnaires have been sent out to monitor the prevalence and burden of disease. Collection is still ongoing. Variables from these forms are now available on request, but they are not yet organized in a way that makes them suitable for disclosure. This makes the data demanding to extract, and it must therefore be noted that delivery can take a long time, especially in cases where a large number of variables are needed. We recommend starting with a smaller number of variables. Please contact the MoBa administration to receive an overview of the variables available.
Ultrasound data
First-generation mothers in MoBa were recruited in connection with the routine ultrasound examination around the 18th week of pregnancy. Hospitals submitted ultrasound data to MoBa for the women who agreed to participate, and most of the data were digitized.

Fertility assessments
Women's fertility
In this survey, we want to learn more about young women's fertility and health. Participants are invited to a clinical examination. This data collection was ongoing up until 2024. The survey was then expanded to also include men (see more information below).

Young women's health and fertility and Young men's health and fertility
Since 2024, "Women's fertility" has included men in the survey in addition to women. Therefore, the name changed to "Young women and men's health and fertility". The aim with this survey is extending the knowledge around health of women and men, the relation between genetics, biology, lifestyle, aging and fertility. The participants were invited to responding to questionnaires as well as clinical examinations.

The Norwegian Environmental Biobank
The Norwegian Environment Biobank monitors levels of nutrients, environmental toxins, and other unwanted substances in the body over time, and investigates how these substances affect our health.
Questionnaire collection 2016/2017 (Part II)
This questionnaire was sent out to mothers and fathers in 2016 in MoBa. Families were also invited to submit samples to the Norwegian Environmental Biobank. Both mother and father received a questionnaire.
The Language and Learning Study (SOL)
The SOL study in MoBa aims to provide more knowledge about the causes and development of language and learning difficulties. The study examines both genetic and environmental factors that may influence children’s language and learning development.
Questionnaire filled out by educational staff in childcare
This questionnaire was sent to some families when the child was 5 years old and contained questions about activities and situations in the childcare centres. The parents were asked to bring the form to the childcare centres to be filled out by the childcare employees. The form was only sent to parents with children born after 2006. Data from the childcare centre questionnaire can currently only be used in co-operation with the Language and Learning Study (SOL). Collection of this questionnaire is complete.
Questionnaire filled out by teacher
Two questionnaires was sent out during the child’s school years. Both were completed by the child’s teacher. The forms contained questions about the pupils learning environment and how the pupil is doing in school and in general. Data from the school questionnaire can currently only be used in cooperation with The Language and Learning Study (SOL). Collection of these questionnaires are complete. These questionnaires only exist in Norwegian. The instrument documentation will soon be available in English.
