This report presents the second national medical record review in Norway, conducted according to the procedure Global Trigger Tool.
Key message
This report presents the second national medical record review in Norway, conducted according to the procedure Global Trigger Tool. The review is coordinated by the national patient safety campaign “In safe hands”. All 19 public health care trusts, and four private hospitals participated. 47 teams reviewed 10,288 patient admissions, randomly selected from the trusts and hospitals throughout 2011. 16 percent of the patient experienced at least one incident of harm. Same as in 2010. 9 percent of the patients experienced at least one harmful incident that led to prolonged hospitalization or more serious consequences (F to I categories). Urinary tract infections, drug-related injuries, post-operative wound infections, surgical complications, and lower respiratory tract infections are the most common medical injuries.
